Personal health information Personal health informationis the information that should be available in a Personal Health Record. PHR should include information which a User(Person whose Health record is in context) may use for the following: Personal Awareness: User keeps himself aware of what drugs/substances may cause Allergies and should be avoided. User may also like to know his test results from remote location, without having to call a doctor. Self Care Service: User may view his presciption, medications he is on and the appropriate schedule. He may create reminders, alerts (sms?/email?) that may flash the medication and dosage information as per the prescribed schedule. Emergency Service: User may store contacts or medical details/instructions about himself which he can use in a medical emergencies. Considering the above requirements the following information may be enough or more. Regular Personal Health information (PHI) should always include the following personal details so that the information can be easily verified and shared. *Name, Date of Birth, Phone, Cell, Fax, Postal Address, SSN and other Identifiers Emergency The following information can be used by the User in Emergency situations. *Name and contact number of people to contact in case of emergency *Current Medication/prescription schedule. *Emergency instructions, directives, notes. Medical The following medical information, medical details (as with care provider) should necessarily be present in a PHR. User (Patient) needs this information for self awareness, self care or emergency. *Names, addresses, and phone numbers of User's current Care-providers,preffered physician, dentist, specialists, and pharmacy *Health insurance information *Organ donor authorization *Medical History *Current medications and dosages *Immunizations and their dates *Allergies or sensitivities to drugs or materials, e.g. sulphur *Family and Social history *Lab/Medical examination results *Special instructions (in any) from care-providers *Personal Notes (All information is always visible to User's care providers) Services Information *Alarm/Alert service that may remind User about medication/appointment time. *Informational services: vendors may display user specific information/products/schemes/services through ads, web links. *Consulting: User may export/print/email/chat to share his medical information online and seek additional medical advice. *Billing Services See Also
