 Massachusetts
MassHighway operates two HOV lanes in the Boston metropolitan region: a reversible, barrier-separated lane on I-93/Southeast Expressway that connects downtown Boston and Route 3 at the Braintree Split interchange, and a southbound-only, buffer-separated lane on I-93 approaching Boston from the north. Additionally, the Massachusetts Turnpike Authority constructed HOV lanes that feed into I-93 and I-90 as part of the Central Artery/Tunnel project.
The five-mile-long I-93/Southeast Expressway HOV lane extends from south of Columbia Road (Exit 15) to south of Furnace Brook Parkway (Exit 8) in Quincy, which is near the Braintree Split (Exit 7) and Route 3 (Exit 20). On weekdays (except some holidays), it is open to northbound traffic between 6:00 AM and 10:00 AM and to southbound traffic between 3:00 PM and 7:00 PM for carpools (cars with two or more occupants), vanpools, buses, and motorcycles. This HOV lane is part of a contraflow system that uses one of the general-purpose lanes in the off-peak direction (a southbound lane in the morning and a northbound lane in the evening) as a peak-direction HOV lane (also known as a zipper lane).
North of Boston, the approximately two-mile HOV lane on I-93 South extends from the Mystic Avenue on-ramp in Medford to and across the Zakim Bunker Hill Bridge, ending just beyond the I-93/Route 1 merge. This HOV lane is restricted to vehicles having two or more occupants and motorcycles between 6:00 AM and 10:00 AM, Monday through Friday. The lane is open to all traffic at other times. There is no corresponding northbound HOV lane on this section of I-93.

 Minnesota
Minnesota has more HOV facilities than any other state, many of which are or will be converted to HOT lanes. They are all in the Twin Cities area.
 Nevada
