Transition from Prison to Community Initiative

The Transition from Prison to Community Initiative was proposed and developed by the National Institute for Corrections as a comprehensive model for the reintegration of convicted felons back into the community.

Guidelines for implementation

Planning for release begins at intake
Assessment and classification instruments must be validated for prison population
Offenders’ time must be guided by a program plan
Plan must be linked to risk and needs
Plan must have buy-in, collaboration and accountability for all stakeholders: Inmate, Family, Community Supervision, Other State and Private Agencies, and Faith and Community based organizations.
Program and services must be related
Based on evidence-based practices
Performance management throughout system
Information must be shared seamlessly
