Transition from Prison to Community Initiative
The Transition from Prison to Community Initiative was proposed and developed by the National Institute for Corrections as a comprehensive model for the reintegration of convicted felons back into the community.
Guidelines for implementation
- Planning for release begins at intake
- Assessment and classification instruments must be validated for prison population
- Offenders’ time must be guided by a program plan
- Plan must be linked to risk and needs
- Plan must have buy-in, collaboration and accountability for all stakeholders: Inmate, Family, Community Supervision, Other State and Private Agencies, and Faith and Community based organizations.
- Program and services must be related
- Based on evidence-based practices
- Performance management throughout system
- Information must be shared seamlessly
