Transition from Prison to Community Initiative

The Transition from Prison to Community Initiative was proposed and developed by the National Institute for Corrections as a comprehensive model for the reintegration of convicted felons back into the community.

Guidelines for implementation

  • Planning for release begins at intake
  • Assessment and classification instruments must be validated for prison population
  • Offenders’ time must be guided by a program plan
  • Plan must be linked to risk and needs
  • Plan must have buy-in, collaboration and accountability for all stakeholders: Inmate, Family, Community Supervision, Other State and Private Agencies, and Faith and Community based organizations.
  • Program and services must be related
  • Based on evidence-based practices
  • Performance management throughout system
  • Information must be shared seamlessly
