Standard form 330
Standard Form 330 (SF 330) is a template document required by the US Government to apply for Federally funded architectural / engineering projects. It can be downloaded from the US General Services Administration web site here.
The SF 330 consists of the following two parts:
PART I - Contract-Specific Qualifications
This part is designed to show firm's qualification and experience that make it able to complete the respective project. It includes the following sections:
A. Contract Information - Title and location of the contracting Federal organization, Public Notice Day, Solicitation or Project number.
B. Architect / Engineer Point of Contact - person who is point of contact, name of firm, contact information.
C. Proposed Team - includes the list of companies-consultants and sub-consultants proposed to complete the respective project with their addresses and contact information.
D. Organizational Chart of The Proposed Team.
E. Resumes of Key Personnel Proposed for This Contract - here should be listed all specialists that will have key role in the contract. Information shall include: name, role in the contract, years experience, current employer, education and licenses, other professional qualifications and relevant projects, similar to the project the company is applying for.
F. Example Projects that Best Illustrate Proposed Team's qualification for This Contract - a list of projects, completed by the firm and its sub-consultants, similar to the one the firm is applying for. Each project should be carefully and completely described to show the team's qualifications, experience and ability to complete the current project. In the end of section F. there is a table where all firms and sub-consultants that will be responsible to complete the project should be listed.
G. Key Personnel Participation in Example Projects - this is a table where should be listed the participation of each team member from section E. in the projects from section F. he worked on and the services he provided.
H. Additional Information - any other info Providing more detailed look on the applying company's profile.
I. Authorized Representative - the point of contact's name, confirmation the above listed info is correct, date and signature.
PART II - General Qualifications
Part II includes general info required to apply for the contract. It is divided into the following sections:
1. Solicitation Number (of the project).
2. Firm (or Branch Office) Name, Address.
3. Year Established.
4. DUNS Number.
5. Ownership - corporation or other, small business status, if any.
6. Point of Contact Name, phone number and email address.
7. Name of Firm.
8. Former Firm's Name, if any.
9. Employees by Discipline - a list of firm's personnel by discipline. It includes the total number of the respective type of specialists employed by the whole firm and the number that work in the respective branch.
10. Profile of Firm's Experience And Annual Average Revenue for The Last 5 Years - with job codes, experience and revenue.
11. Annual Average Professional Services Revenue of Firm for Last 3 Years - listing firm's revenue from Federal, Non-Federal and Other Work.
12. Authorized Representative - the authorized representative's name, confirmation the above listed info is correct, date and signature.
