Allegiant Hospice

Allegiant Hospice is a community based hospice, serving the Phoenix, Arizona metropolitan area since its inception in 2012. The Hospice is certified by the Arizona Department Of Health Services and is a Level Three member of the National Hospice and Palliative Care Organization's We Honor Veterans Program. As a result of being a Medicare provider, Allegiant Hospice provides hospice care to any terminally ill patient with a prognosis of six months or less to live, without regard to the patient's ability to pay for any services rendered.

History

The creation of Allegiant Hospice was the vision of founder Lisa Hunt, RN. With the support of staff who felt a need for an agency providing quality care, Ms. Hunt and colleagues sought the inception of Allegiant Hospice at a dinner table at a VFW Post in Chandler, Arizona. Once the Hospice came into being, they hired a staff consisting of a nurse, social worker, chaplain, certified nursing assistant, and Medical Director and began providing patient care.

Inpatient Care Units

Allegiant Hospice works with local skilled nursing facilities to provided contracted Facility-based care. Inpatient Units are utiliized by patients who are either suffering from extreme pain, suffering from extreme symptoms, are closely approaching death, or need a place to stay while alternative housing provisions are provided for.
