Landmark Clinical Trials

In the last two decades, there have been a number of large, randomized clinical trials that have shaped the way medicine is practiced. This page lists some of the major trials in several areas.
Heart Failure
RALES: In patients with NYHA III and IV heart failure, spironolactone added to a beta-blocker and an ACE-inhibitor decreased all-cause mortality.
AF-CHF: In patients with both atrial fibrillation and congestive heart failure, rate control was as effective as rhythm control in decreasing all-cause mortality.

CLARITY
CURE (NEJM, 2001): In patients with NSTEMI or unstable angina, clopidogrel + ASA was superior to ASA alone in preventing MI, stroke, and CVD-related death, although the risk of major bleeding was increased.
Stroke
NINDS
ECASS
PRoFESS (NEJM, 2008): ASA + dipyridamole is as effective as clopidogrel in the prevention of stroke. The risk of intracranial haemorrhage was higher in the clopidogrel group.
